Get Well Clinic
Privacy Policy
Get Well Healthcare Services, LLC, doing business as Get Well Clinic (“Get Well Clinic,” “we,” “us,” or “our”), respects your privacy and is committed to protecting your personal and health information. This Privacy Policy explains how we handle information in connection with our website at www.getwellclinic.com (the “Site”) and the in-person and telehealth services we provide to patients in the State of Texas.
We provide care to Texas patients only. We are a self-pay family practice offering everyday medical care and medical weight loss services, delivered in person at our Texas clinic locations and by telehealth.
1. Our commitment to your privacy
As a healthcare provider, Get Well Clinic is a “covered entity” under the federal Health Insurance Portability and Accountability Act of 1996 and its implementing regulations (collectively, “HIPAA”). We protect your individually identifiable health information (“Protected Health Information,” or “PHI”) in accordance with HIPAA and applicable Texas law, including the Texas Medical Records Privacy Act. The way we use and disclose your PHI in connection with your care is described in our Notice of Privacy Practices, which is incorporated into this Policy by reference.
2. Information collected through this website
This Site is an informational, brochure-style website. We do not collect personal information directly through this Site. Specifically:
- The Site does not contain any intake forms, contact forms, or document/ID upload tools.
- The Site does not use advertising or marketing tracking pixels (such as Meta/Facebook, Google Ads, or TikTok pixels).
- The Site does not use analytics tools or non-essential cookies to track you.
When you choose to book an appointment, access the patient portal, or contact us, you leave this Site and interact with our service providers (described in Section 4) or contact us directly by phone or email. Any information you provide at that point is governed by this Policy and our Notice of Privacy Practices.
As with most websites, our hosting provider may automatically log limited, standard technical data (such as IP address and browser type) for security and to keep the Site running. This information is not used to identify you and is not combined with your health information.
3. Information we collect when you become a patient
When you book an appointment, visit one of our clinics, or use our telehealth services, we collect information needed to provide care and operate our practice, which may include:
- Identifying information — such as your name, date of birth, address, phone number, and email address.
- Health information — such as your medical history, medications, symptoms, vital signs, treatment plans, and other clinical information you or your providers share.
- Payment information — such as the card or payment details used to pay for self-pay services. Card data is processed by our payment processor and is not stored on this Site.
4. How your information is used and shared
We use and share your information for treatment, payment, and healthcare operations as permitted by HIPAA, and to provide the services you request. We share information only with service providers and partners that help us deliver care, and only as needed. These include:
- Calendly — used to schedule appointments.
- Practice Fusion / Patient Fusion — our electronic health record system and patient portal.
- Empower Pharmacy — used to fill and ship prescriptions when you choose a shipped option.
- Our payment processor (Clover) — used to securely process self-pay card payments.
Where these partners create, receive, maintain, or transmit PHI on our behalf, they act as our “business associates” under HIPAA and are required by written agreement to safeguard your information.
We may also disclose information when required or permitted by law — for example, to public health authorities, in response to a valid legal order, or to prevent a serious threat to health or safety.
5. We do not sell your information
We do not sell, rent, or trade your personal or health information to anyone. We do not use your PHI for third-party advertising, and we do not share your health information with advertising networks.
6. How we protect your information
We maintain administrative, physical, and technical safeguards designed to protect your information against unauthorized access, use, or disclosure. Our electronic health record and patient portal are operated by HIPAA-compliant vendors that use encryption and access controls. While no system can be guaranteed to be perfectly secure, we work to protect your information consistent with HIPAA requirements.
7. Your rights regarding your health information
Under HIPAA and Texas law, you have rights regarding your PHI, including the right to:
- Inspect and obtain a copy of your medical records;
- Request a correction or amendment to your records;
- Request an accounting of certain disclosures of your PHI;
- Request restrictions on how your information is used or shared;
- Request confidential communications; and
- File a complaint if you believe your privacy rights have been violated.
These rights, and how to exercise them, are described more fully in our Notice of Privacy Practices. To make a request, contact us using the information in Section 10.
8. Children’s privacy
This Site is intended for a general adult audience and is not directed at children under 13. We do not knowingly collect personal information from children through this Site. When we provide care to minor patients, their health information is handled in accordance with HIPAA and Texas law, and with appropriate consent from a parent or legal guardian.
9. Changes to this Privacy Policy
We may update this Privacy Policy from time to time. When we do, we will revise the “Last updated” date above and post the current version on this page. Your continued use of the Site after changes are posted means you accept the updated Policy.
